Simple Referral Process
VIBEcare offers a streamlined, evidence-based model for integrated behavioral health that fits naturally into primary care workflows. Primary Care Providers refer patients directly through their electronic health record (EHR).

Fast Access to Virtual Care
Within two business days, patients are contacted and scheduled for virtual sessions with a Board-Certified Health Coach. Care focuses on common behavioral health conditions, including depression, anxiety, substance use, insomnia, and related concerns.

Ongoing Care Coordination & Monitoring
Care is coordinated through VIBEcare’s HIPAA-secure technology platform, allowing for consistent follow-up and progress tracking using a patient registry. This structured approach supports continuity of care while reducing administrative burden for primary care teams.

Provider Communication & Clinical Support
Primary Care Providers receive monthly progress reports through the EHR and may consult with the Health Coach as needed. Psychiatric consultation is available to support care decisions, ensuring coordinated, clinically appropriate treatment while keeping the PCP informed and involved.
